Please submit an inquiry through our Contact page and our team will get you started with our Requisition Form. Ensure that your patient signs and dates the Consent Agreement.

* Please note that we require a doctor’s referral in order to participate in our laboratory testing. To ensure proper medical attention and result interpretation, Health Genetic Center requires all samples for DNA testing to be submitted by physicians only.

Test Selection

The doctor fills out information about the clinic and the patient on the Requisition Form and selects the required test(s) or panel(s) (see DNA Tests and DNA Panels for more information).

Sample Collection

Collect samples for testing or arrange collection at a third party laboratory. Doctor may collect any number of samples for the required test(s) either using our recommendation on the Requisition Form. Please note the we do not arrange sample collection for our testing. Doctor may refer to the Standard Protocol for Sample Collection at a 3rd Party Lab, for general guidelines.

Sample Delivery

Most third party laboratories have a shipping service that can be used to send samples to our testing laboratory. We recommend sending samples overnight or within three days with an ice pack. This also means that samples are best collected between Monday and Wednesday to compensate for possible delivery delays. If your patient prefers and/or the collection laboratory allows, samples may be dropped off by the patient during our office hours (M-F, 9am to 8pm or Saturdays 10am to 3pm.)

Results

DNA test results are reported within 10 working days from receipt of samples and will be sent directly to the doctor's office.
